PLEASE READ THE FOLLOWING STATEMENTS CAREFULLY BEFORE SIGNING THIS METALTECH PRODUCTS INC. (MPI) APPLICATION FOR EMPLOYMENT. ONLY THOSE APPLICATIONS THAT ARE SIGNED AND DATED ARE CONSIDERED VALID. IF YOU HAVE ANY QUESTIONS REGARDING THIS STATEMENT, PLEASE ASK THEM BEFORE SIGNING.


NOTICE OF PRE-EMPLOYMENT AND RANDOM DRUG TESTING POLICY
For the safety of its employees, MPI strives to maintain a drug-free workplace. If MPI makes you a conditional offer of employment, you will be required to take a mandatory pre-employment drug test at the company’s expense. In the event you are hired, you will be subject to random drug testing at any time during employment.


NOTICE OF SKILL ASSESSMENT(S)
Prior to consideration for employment, MPI requires all prospective employees to submit to a written skill assessment(s). This written skill assessment(s) will be used to help evaluate your ability level to perform the skills for the job(s) you are applying for. If MPI makes you a conditional offer of employment, you may be required to submit to a hands-on skill assessment(s).


AUTHORIZATIONS/ACKNOWLEDGMENTS
I authorize Metaltech Products Inc. (MPI) to investigate all statements contained in this Application for Employment as they may deem necessary in arriving at an employment decision. I authorize any of the persons or organizations named in this Application for Employment to release any verbal and/or written information concerning records regarding my employment, my education, my character, my qualifications, etc., and I release all parties from liability for any damage that may result. I acknowledge that if I become employed by MPI I will be responsible for familiarizing myself with all policies, procedures, rules, regulations, etc. of the company as they presently exist, or are later modified. I also acknowledge that if I become employed by MPI, my employment and compensation can be terminated at the discretion of the company, or at my option, without notice at any time. I understand that any employee rules, policies, benefits, or other statements, whether oral or written, made by the company or its representative, ARE NOT contracts of employment between the company and me. I also understand that no representative of the company has any authority to enter into any employment agreement for any specified period of time, or to assure me of any future position, benefits, or terms and conditions of employment, except as specifically stated in a current written agreement signed by the President. I acknowledge and accept that falsifying, misrepresenting, or omitting any material facts in company employment documents may result in termination at any time. I acknowledge that this Application for Employment is not an offer of employment, and no promises or representations of employment have been made to me at this time.

NOTICE BEFORE ORDERING CONSUMER REPORTS
(Including Motor Vehicle Reports and Credit Reports)

The Fair Reporting Act (FCRA) provides individuals with certain rights regarding consumer reports and places certain obligations on employers using consumer reports for employment-related purposes. Consistent with the FCRA’s requirements, this notice is provided to you in order to inform you that Metaltech Products Inc. (MPI) may, for employment-related purposes (e.g., evaluating you for initial employment, promotions, transfers, assigned duties, retention as an employee, etc.), obtain from a consumer reporting agency one or more consumer reports containing criminal record information, driving record information, and/or other relevant information about you. MPI will not obtain a consumer report without your signature below authorizing us to obtain one or more consumer reports. It is the policy of MPI not to obtain any consumer report unless the report, in some way, relates to the employee’s job description or job performance.

AUTHORIZATION TO OBTAIN CONSUMER REPORTS

I hereby acknowledge that I have read, understood, and accepted the contents of the above notice, and by signing below, I specifically authorize Metaltech Products, Inc. (MPI) to obtain one or more consumer reports on me for employment-related purposes as indicated above.
